Our Care Coordination department brings together several services designed to meet the full spectrum of our patients needs and address the social factors that influence health outcomes, such as access to safe housing, reliable food, and other essential resources.

Health Home

Through our Health Home program, patients with Medicaid who are managing multiple chronic conditions or behavioral health needs receive a dedicated Care Manager. This person partners with you and your entire care team including primary care, specialists, behavioral health providers, and community supports to coordinate services and create a plan tailored to your needs.

Patient Navigation

Our patient navigators help you move through the health care system successfully. They assist with scheduling, follow up, specialist coordination, referrals, and removing barriers that make care difficult to access. Navigators also help connect patients to community resources that support stability, which research shows can lead to better overall health.

Language Services

In addition to phone interpretation, a core part of our mission at Jericho Road is our live interpretation services. Our team of medically certified interpreters ensures that patients receive information and care in the language they understand best so communication is clear, accurate, and culturally respectful at every step of the process.

Community Health Workers

Our Community Health Workers connect patients to resources in the community. They provide outreach, education, support, and link individuals to services such as food programs, housing assistance, transportation, and other needs that directly affect health. Because challenges like housing instability and food insecurity are linked to higher rates of chronic disease complications and increased hospital use, addressing these needs is a key part of our approach.
