Every day, our Care Coordination team partners with individuals and families across Buffalo to navigate healthcare systems, connect to resources, and build pathways toward long-term stability.
Care Coordinators walk alongside patients, helping them:
- Understand and follow their care plans
- Coordinate appointments and referrals
- Access transportation, insurance, and social supports
- Strengthen their own capacity to thrive

2025 Holiday Drive
Looking for another way to contribute?
This holiday season, we’re also collecting food, hygiene, and winter kits to share through our Care Coordination team.
Item Needs:
- Food Kits – Shelf-stable, nonperishable foods that can sustain an individual or family for a few days.
- Hygiene Supplies – Soap, shampoo, toothpaste, deodorant, lotion, and toilet paper.
- Winter Kits – Gloves, hats, socks, and other warm essentials for Buffalo’s coldest months.

Donations can be dropped off at 181 Barton St, Buffalo NY on:
12/2 – 12:00-4:00pm
12/16 – 10:00 – 2:00pm
