Jericho Road takes all private and government insurances. Please bring a copy of your insurance card to your appointment. For patients with an HMO, a Jericho Road provider must be your assigned PCP with your insurance plan.

Please don’t let a lack of insurance keep you from seeing us. If you do not have insurance, we have several options to help you. We can provide assistance in applying for public medical assistance programs. Patients who do not qualify for these programs can receive a medical visit for as little as $20, based on your income and family size through our sliding fee discount program.

Jericho Road’s Sliding Fee Discount Program
Jericho Road Community Health Center welcomes all patients and insurance plans. Should you need assistance with medical coverage, please inquire with a front office receptionist about our sliding scale fee, which is available upon request. No one will be denied access to services due to inability to pay. There are discounts available based on family size and income. Jericho Road will base program eligibility on income and household size only, and will not discriminate on the basis of age, gender, race, sexual orientation, creed, religion, disability, or national origin. The Federal Poverty Guidelines will be used in creating and annually updating the sliding fee schedule (SFS) to determine eligibility.

To apply for our sliding fee discount program, please review the sliding fee discount program policy and additional information, then fill out the sliding fee discount program application and bring it to your provider’s office.

While we are committed to making care affordable for all our patients, you will need to pay your copay at the time of your visit. Paying a little now may save you big bills in the future. We accept cash, check, and credit card. Not having payment at the time of service may result in your appointment being rescheduled.
